NOR/CFI: use bus_width for memory access on flash ID.
NOR flash structure requires each access to be bus_width wide. Fix read of flash ID accordingly to rule above. Add case (chip_width == 4), allowed by CFI spec and coherent with current value of CFI_MAX_CHIP_WIDTH but currently not used by any target.
