For consistent simulation behavior it is recommended to annotate all source
files with a timescale. Add it to those where it is currently missing.

Currently the individual IP core dependencies are tracked inside the
library Makefile for Xilinx IPs and the project Makefiles only reference
the IP cores.
For Altera on the other hand the individual dependencies are tracked inside
the project Makefile. This leads to a lot of duplicated lists and also
means that the project Makefiles need to be regenerated when one of the IP
cores changes their files.
Change the Altera projects to a similar scheme than the Xilinx projects.
The projects themselves only reference the library as a whole as their
dependency while the library Makefile references the individual source
dependencies.
Since on Altera there is no target that has to be generated create a dummy
target called ".timestamp_altera" who's only purpose is to have a timestamp
that is greater or equal to the timestamp of all of the IP core files. This
means the project Makefile can have a dependency on this file and make sure
that the project will be rebuild if any of the files in the library
changes.
This patch contains quite a bit of churn, but hopefully it reduces the
amount of churn in the future when modifying Altera IP cores.

This reduces the amount of boilerplate code that is present in these
Makefiles by a lot.
It also makes it possible to update the Makefile rules in future without
having to re-generate all the Makefiles.

In cases when a shallow FIFO is requested the synthesizer infers distributed RAM
instead of block RAMs. This can be an issue when the clocks of the FIFO are
asynchronous since a timing path is created though the LUTs which implement the
memory, resulting in timing failures. Ignoring timing through the path is not a
solution since would lead to metastability.
This does not happens with block RAMs.
The solution is to use the ad_mem (block RAM) in case of async clocks and letting
the synthesizer do it's job in case of sync clocks for optimal resource utilization.
Make sure that the right hand side expression of assignments is not wider
than the target signal. This avoids warnings about implicit truncations.
None of these changes affect the behaviour, just fixes some warnings about
implicit signal truncation.

Move the CDC helper modules to a dedicated helper modules. This makes it
possible to reference them without having to use file paths that go outside
of the referencing project's directory.

All the hdl (verilog and vhdl) source files were updated. If a file did not
have any license, it was added into it. Files, which were generated by
a tool (like Matlab) or were took over from other source (like opencores.org),
were unchanged.

Currently the BRAM and data registers in the util_axis_data are ungated
when the FIFO is ready to receive data. This good for high-performance
since it reduces the number of control signals. But it is bad from a power
point of view since it causes additional reads and writes.
Change the core gate the BRAM and data register if either the consumer is
not ready to accept data or the producer has no data to offer.

Re-generate the Makefiles after a small update to the generation script:
- Entries are sorted alphabetically
- Empty dependency lines are skipped

Clean ran for a project will clean only the project files.
Clean-all ran for a project will clean also the library files on which the project depends.
The clean commands will only remove specific files and directories.
The top Makefile allows several options(per suggestion from jameyhicks):
make fmcomms1.zed will run "make all" in projects/fmcomms1/zed/
make clean will run "make clean" for all the projects
make clean-all will run "make clean" for all the projects and libraries
make lib will run "make all" for the library files
Add a room output on the input side that reports how many free entries the
FIFO has and a level output on the output side that reports how many valid
entries are in the FIFO.
Note that the level output is only accurate if the output of the FIFO is not
registered, otherwise it might be off by one.

Add a option to specify whether the FIFO should have a registered output
stage or not. This is useful if the user wants to implement that stage
itself.

Bring back the AXIS FIFO as a separate module instead of embedding it into
the DMAC module. This makes it possible to use it in other modules outside
of the DMAC.
